Are you using the new Form 1 from ESE?

On February 7, 2017, the Department of Elementary and Secondary Education (ESE) Office of Approved Private Special Education Schools (OASES), formerly Program Quality Assurance (PQA), sent out a notice regarding changes to ESEs Form 1 –Notification/Request for Prior Approval of Substantial Changes within a Private or Public Special Education School Program. There is one major change to the Form 1 which is a 10% decrease in student enrollment or the prior approval of a 10% increase in student enrollment is based on the last approved ESE student enrollment.

In addition, the instructions on the Form 1 have also been updated to include uploading the Form 1 into the Web Based Monitoring System (WBMS). 

In the next 2-3 weeks ESE will host a webinar to review how Form 1’s and Form 2’s (Private or Public Day or Residential School Program Incident Report) will be uploaded and processed. Until then, please continue to mail the Form 1’s and Form 2’s to ESE.
